There is a side of therapy which has nothing to do with muscle coordination or retraining the brain to do daily tasks of communication skills. It’s the human side of therapy – the side of therapy that gives people hope, making them believe they can perform activities which they might have thought were never again possible. It’s all about recovery and a sense of personal accomplishment.

Our passionate team includes occupational therapy, physical therapy, and speech therapy. What sets us apart from other facilities is that with all of our years of experience we offer very extensive individualized treatment plans that not only includes our recommendations but your input as well. Greenhaven Health and Rehabilitation Center’s close relationship with United Healthcare and Evercare helps our community also stand out from the others. With their one-on-one approach with the families, staff and resident, we provide a higher quality of care for our residents.

For many families, the search for quality, compassionate care for their aging loved ones can be emotionally draining. Nursing home residents often need specialized care and support. Evercare® Nurse Practitioners serve as health care advocates and provide an extra level of care management for residents in skilled nursing facilities. They work with the resident, their doctor, family and nursing home staff to help create a custom care plan that reflects their preferences and wishes.

Nurse practitioners are the heart of our program and develop a one-on-one relationship with residents to help them achieve their highest level of health and independence. The Evercare Nurse Practitioner also acts as a communication link to keep family and loved ones informed and involved.

UnitedHealthcare® Medicare Advantage plans are insured through UnitedHealthcare Insurance Company and its affiliated companies, a Coordinated Care plan with a Medicare contract. This plan is available to anyone with Medicare living in a contracted nursing home.
